From the European Union to Catalonia to receive treatment |

 U citizens who come to Catalonia to receive a particular (scheduled) treatment within the public health service must previously have received the
E 112 form entitled "Certificate concerning the retention of the right to sickness or maternity benefits". |
The E112 form states the specific treatment, the address of the health centre or service to provide the treatment and the period during which it will be provided. This form must be issued by the relevant authorities in your country of origin. |

Before leaving home, you must make a reservation in the relevant hospital in Catalonia.
