Requesting a TSI |
I have never had a TSI, and I would like one now. |

· If you are registered as living in Catalonia, you should make the request at your primary health care centre (CAP), where you will be registered and assigned a general practitioner. You will also be given a provisional identification document (PID) while your request for a TSI is being processed.

| Do dependants have to go personally to request a TSI? |

· Anyone can go to the CAP to request a card for another person, but they must take documents showing the person is registered with Social Security and their National Identification Document (DNI). You may also be asked for proof of being registered to live in the area.

| Can I request my TSI from the CAP closest to my work, even though it corresponds to a different area than the one I live in? |

· No. You must go to your local CAP.

| Can I request a TSI over the Internet? |

· Only in the case of asking for a new card in the event of losing the original one, to change information, because the condition of the card has deteriorated or the magnetic strip does not work.
· You can also make the request online if you asked your CAP for it but it has not arrived. All online requests can be made from the OAC website. www.cat365.net.
