Primary health care is citizens' first level of access to the health care system. Primary health care services are available at your primary health care centre (CAP), where teams of health professionals are working to help you. |

 our primary health care centre (CAP) is the first place you should go when you have a health problem or want to prevent a problem. In smaller towns, the visit is made at local surgeries.

| We integrate preventive and curative health, rehabilitation and community health promotion in a balanced manner. |

Your CAP is equipped with a HQPT, or highly qualified professional team, to help guide you in all your social and health care needs. |

You have your own health care team
At your CAP, you can establish a relationship of trust with your own team of health care professionals. The health care professionals that work at your CAP (medical and nursing staff) are always nearby to help you and your family. You can establish a personalised relationship of trust with them and they can advise you on all health-related matters.
At your CAP, your general practitioner can help you stay healthy and well. Your doctor will offer ongoing assistance and resolve any incident you consider urgent.
At your CAP you can find
· A team of health care professionals who will always be ready to help you and your family.
· Personalised treatment you can trust.
· Information and advice on health-related matters.
· Home visits for people whose state of health means they cannot get to their CAP.
· Ongoing after-hours care, or information on the ongoing care centre closest to you.
· Preventive measures (vaccinations, early detection programmes, etc.) and promotion of healthy habits and advice.
· The User Service Unit (USU).
